This is not a preference or an aspirational model: it is a clinical necessity that responds to the biopsychosocial complexity of each patient and the technical responsibility of each intervention.

In these surgeries, each decision has implications that cross multiple dimensions: endocrinological, psychiatric, functional, legal and social. To try to operate without contemplating this clinical map is to ignore the fact that the body that arrives in the operating room is not only an anatomy, but a journey. And that journey demands to be listened to and validated by more than one specialty.

The multidisciplinary team is not a sum of knowledge, but an active clinical structure. It allows sharing diagnostic criteria, discussing indications, anticipating complications and articulating responsibilities. In this model, the surgeon does not act alone: he coordinates with mental health professionals who evaluate emotional stability and expectations, with endocrinologists who regulate hormonal treatments according to perioperative risks, and with clinicians who guarantee integral medical aptitude. In many cases, social assistance, legal or gynecological counseling are also involved, depending on the type of transition.

These structures are not only desirable: they are effective. A systematic review published in JAMA Network Open (2020) analyzed the impact of multidisciplinary teams in the management of surgical patients and concluded that their intervention is associated with better clinical outcomes and lower incidence of complications (source). In a complementary manner, another study by JAMA Surgery (2023) showed that the reimplementation of surgical checklists - key elements in collaborative systems - improves team performance and postoperative safety (source). The evidence supports what clinical practice already reveals: operating in a network is not just an ethical aspiration, it is a strategy that saves tissue, time and lives.

The endocrinologist plays a key role in gender affirmation surgeries, especially in perioperative hormonal management. Although it was previously recommended to suspend estrogens due to thromboembolic risk, recent studies show that their continuation does not significantly increase this risk in procedures such as vaginoplasty (PubMed, PMC). These decisions must be made on a case-by-case basis, and only effective coordination between endocrinology and surgery can balance medical safety and respect for gender identity.

From a mental health point of view, preoperative assessment is not limited to diagnosis, but is articulated with emotional accompaniment, desire support, validation of autonomy and the construction of informed consent as a process. According to WPATH data in its SOC8 (2022), centers applying full psychosocial criteria report rates of post-surgical regret lower than 1% (see document). Clinical ethics does not only require listening: it requires that what is heard has structural consequences.

Accumulated evidence from public hospitals and private centers with sustained case volume confirms that functional and esthetic outcomes improve significantly when working with structured teams. Revision surgeries are reduced, self-perceived quality of life is improved and long-term clinical bonds are strengthened.

Transgender medicine cannot be sustained on unipersonal models. It needs structures capable of providing sustained, technically solvent and ethically legitimized support. Because the integral is not an abstract ideal: it is a form of clinical practice that combines technical precision with human respect.

En la práctica quirúrgica contemporánea, pocas áreas requieren un abordaje tan interdisciplinario como la cirugía de afirmación de género.
