What does phalloplasty consist of?
There is no universal "ideal technique". The phalloplasty is a set of customized surgical procedures that are performed in stages, with the objective of:
Build a neophallus with natural appearance, volume and position.
Allowing urination in an upright position (if desired)
Incorporate erotic sensitivity
Enabling penetration by means of erectile prosthesis
Every patient is different, which is why the choice of technique depends on your anatomy, expectations and functional and esthetic needs.
Surgical techniques used
Pedicle Flaps
They are safer options when using local tissue. They consist of sliding skin and fat from neighboring areas to form the penis:
Suprapubic flap (lower abdomen)
Gluteal or thigh flap
Vascularized Free Flaps
They are more complex and allow better esthetic and functional results. They involve microsurgical transplantation of tissue with nerves and vessels:
Radial forearm flap (widely used)
latissimus dorsi flap (back)
Each option has advantages and limitations that will be explained in consultation.
Phalloplasty Stages
Reconstruction is performed in multiple surgical phases, with 6-month intervals between each one:
Construction of the neophallus: volume, shape and position are defined. It may or may not include urethra.
Extended urethra formation: allows standing urination. Not all patients choose this option.
Implantation of penile prosthesis: to achieve erections. It is performed only if the patient desires it.
Testicular prosthesis implantation and scrotal modeling.
